Directions for Final Forms (new platform for Physicals) & Team App
For the 2023-2024 there will be some changes to some of the programs we use.
Final Forms~ This is a new VHS athletics on-line platform for how we manage our IHSAA required physicals. You will no longer turn physicals in to the athletic office. Instead, everything is uploaded and processed on-line. This streamlines how we get our information and use it for the coaches and the athletic department. YOU will create an account first and then upload your physical either through pictures with your phone or a scanner. You will also sign the required documents that the athletic department and the IHSAA require through final forms. Downloadable directions are below.
Team App~ This year we have switch communication applications from Remind to Team App. Team app is an application that is used by multiple sports teams here at VHS. It allows us to communicate with parents and athletes as well as have access to results, scores, schedules, etc. Please follow the steps in the downloadable directions below to send a request to be approved to the team Valparaiso Swimming & Diving.
