Directions for Final Forms~platform for Physicals

We will be using Final Forms again for the 2024-2025 school year. It is an online platform for how we manage our IHSAA-required physicals. You will no longer turn physicals into the athletic office. Instead, everything is uploaded and processed online. This streamlines how we get and use our information for the coaches and the athletic department. You will create an account first and then upload your physical through pictures with your phone or a scanner. You will also sign the required documents that the athletic department and the IHSAA require through final forms. If you have questions, please look at the downloadable directions below for the final form or contact the athletics department. A physical must be completed before any IHSAA Limited contact or official practice.
